Placement evidence checklist
Prepare worker evidence for the next placement.
Agree what the assignment needs, who reviews it and what the client receives before the worker starts.
Start the checklistDefine the assignment with the client.
Record the role, workplace, planned dates and evidence the client requires for this assignment.
Identify which requirements apply to this placement rather than assuming every client needs the same documents.
Agree who conducts each check.
Name the organisation and reviewer responsible for each required check. Record the method and where its outcome will be held.
Discuss agency and client responsibilities before collecting documents. A checklist does not decide those responsibilities for you.
Ask the worker for relevant evidence.
List the personal documents and credentials needed for the role. Check what the worker has shared and what is still missing.
Previous evidence gives your reviewer context. It does not remove checks required for a new role or employer.
Review evidence against the assignment.
Inspect the evidence, source, relevant dates and required check results. Record the decision against this placement.
Keep supplied documents separate from completed reviews and from information the agency maintains.
Confirm what the client can access.
Agree the information the client needs and how it will be shared. Check access before sending the placement record.
Share the relevant evidence and review information through the agreed process. Keep unrelated personal documents private.
Plan follow-up work between assignments.
Record the next review date and the action it triggers. Name who will request updated evidence and who will review it.
Before the next placement, compare the evidence with that client’s requirements again.
Follow the checklist into the working record.
